We exist to strengthen primary care, ensuring dignity, access, and lasting impact for communities today and for generations to come.
As we reflect on 2025, we are reminded that meaningful change in health does not begin in hospitals or specialised centres—it begins in communities. This conviction continues to guide our work as we strengthen our focus on primary care and our role as a trusted front door for underserved communities navigating complex healthcare systems. Primary care is where continuity, dignity, and trust are built, and it remains one of the most effective ways to improve health outcomes equitably and sustainably.

This year marks a renewed commitment to extending our reach to underserved populations facing structural, financial, and social barriers to care—ensuring no community is rendered invisible by conventional systems.
Why This Matters?
Financial constraints and gaps in the primary healthcare system prevent low-income communities without universal health coverage from accessing care leading to poor health-seeking behaviours and adverse outcomes, particularly for maternal and child health.
Among the underserved communities we served in the last five years, pregnancy-related complications and newborn complications has been the top two medical conditions that require referral to tertiary care for life-saving treatments.

Our Theory of Change
Poor maternal and child health outcomes during the first 1,000 days such as malnutrition, low birth weight, and delayed development can entrench cycles of poor health and poverty across generations.​

Autrui365 – Continuity of Care Across Life
Autrui365 is our continuity-of-care framework, designed to support individuals and families before, during, and beyond programme-specific interventions such as the First 1,000 Days. It ensures that health gains achieved in early life are maintained and protected over time, while also providing access for those outside the maternal-child window.

What's Next?
As we continue to strengthen primary healthcare for underserved communities, we are expanding our work to Penang and bringing The First 1,000 Days Project to the north.
